Registration Form –

You must Register and pay for all programs. Each participant is required to provide his/her own soccer shoes, shin guards, soccer ball and other equipment as indicated by each program.  Uniforms are not included with your registration payment, unless otherwise specified.  If you have any questions about any of the programs, contact Kick it Right to (312) 622-1638, or E-mail: registration@kickitright.com
